Six is a small child in a yellow raincoat, alone inside the Maw — a vast, rusted vessel deep below the sea. She is hungry. The Maw is full of enormous, distorted figures who would be happy to find her. She must sneak, climb and solve her way through the ship's many horrifying layers to reach the surface before hunger becomes the least of her problems.

Little Nightmares is a 2.5D puzzle-platformer horror game built around the perspective of a tiny child in a world scaled to adult proportions — which makes even an ordinary kitchen terrifying when the inhabitants are twice the size of a door. The art direction is its defining achievement: a handcrafted aesthetic of rotting wood, slick surfaces and grotesque character designs that evokes a half-remembered nightmare about being very small. Each chapter of the Maw introduces a new environment and a new grotesque adversary — the long-armed Janitor, the lumbering Twin Chefs, the spectral Lady — each requiring a different approach to survive. Six has no combat ability; stealth, observation and speed are her only tools. The story is told entirely without dialogue or text, through environment and implication. The Secrets of The Maw DLC expansion follows a different character — a small gnome-like child called the Runaway Kid — through unexplored areas of the ship.
