An enormous alien structure appears in Earth's orbit without warning. Humanity calls it the Metahedron. The Sentient Contact Assessment and Response team — SCAR — is assembled and launched to investigate. The mission goes wrong immediately, and Dr Kate Ward wakes up alone on an unknown extrasolar planet with no clear way home and an environment that wants her dead.

Scars Above is a third-person action shooter built around a scientist protagonist in a hostile alien world rather than a trained soldier. Kate approaches the planet's dangers with the tools of a researcher: studying enemy weaknesses, combining elemental attacks to discover reactions, and recording observations in her journal as the planet's biology and structures gradually reveal their nature. The combat system rewards preparation and understanding — enemies have elemental vulnerabilities that vary and shift, and successfully chaining a freeze followed by an electric discharge, or applying plasma to a wet target, deals dramatically more damage than brute force. Stamina management and dodge timing give encounters a souls-adjacent structure, with predictable boss attack patterns rewarding patience. The game places storytelling through environmental details and research logs alongside its action systems, building a coherent alien world through Kate's scientific perspective.
