France, 1348. The Black Death has arrived. Behind the plague comes the Inquisition, searching for a child. Amicia de Rune is fifteen. Her brother Hugo is five. Their parents are dead. The road ahead is through a country that is dying in two directions at once.

A Plague Tale: Innocence is a linear narrative action-adventure in which the player guides Amicia through plague-ravaged medieval France, protecting her younger brother Hugo from both the Inquisition soldiers pursuing them and the rat swarms that have consumed the countryside. Amicia carries an alchemic sling loaded with different types of stones and compounds: ignifer creates fire that holds the rats at bay, extinguis extinguishes torches to release the swarm onto enemies, and odoris attracts rats toward a target. Managing light and darkness is the central mechanic, since rats cannot cross lit areas but light attracts human enemies. Stealth and problem-solving drive most encounters, with direct combat available but dangerous. The relationship between Amicia and Hugo is the narrative backbone of every scene, and the game's emotional weight comes from the accumulating cost of what they are forced to do to survive. The sequel, A Plague Tale: Requiem, continues their story directly.
