A parasitic worm called the Devil Worm has infected most of humanity, transforming carriers into monstrous creatures called the Ridden. The few who are immune — the Cleaners — have banded together at Fort Hope, one of the last functioning human settlements. You are one of them. Your mission is to push out into Ridden territory, clear infestations, rescue survivors and keep humanity's last stronghold alive.

Back 4 Blood is a co-op first-person shooter built by the team behind the original Left 4 Dead, and shares its DNA: four-player teams push through gauntlets of infected in third-person campaign levels, with tougher Special Ridden mutations ambushing from unexpected angles. The distinctive addition is the card system — before each run, players build decks of Supply Cards that buff stats, add abilities and shape playstyle, while the AI Game Director draws Corruption Cards that add difficulty modifiers, environmental hazards and special enemy spawns to the level. No two runs feel identical. Eight playable Cleaners each bring passive bonuses and personality. PvP Swarm mode puts a team of Ridden players against a team of Cleaners in enclosed arenas. Three paid DLC expansions and free updates were released through 2022.
