Investigative journalist Blake Langermann and his wife Lynn crash-land their helicopter in the remote Arizona desert while investigating the murder of a pregnant woman. Lynn disappears into the darkness. Blake recovers with only his camera and must find her — moving through Temple Gate, a hidden settlement built by the doomsday cult of Sullivan Knoth, who believes the apocalypse is moments away and that the child Lynn carries may be the Antichrist.

Outlast 2 is a first-person survival horror game with no combat and no weapons beyond a camcorder. Like its predecessor, the entire experience is built around hiding, running and enduring. Blake's camera night-vision mode illuminates the dark but drains the battery, forcing constant management of limited camera batteries found while fleeing. Temple Gate is an open, sprawling environment spanning cornfields, irrigation ditches, a school and the settlement itself, with two enemy factions — Knoth's followers and a rival sect called the Heretics — both hunting Blake simultaneously. Interwoven throughout are flashback sequences set in Blake's old school, shot in a different visual register that deepens the psychological and religious dimensions of the story. The game is notably more disturbing and thematically extreme than the original, dealing directly with religious mania, violence and grief. Sequel The Outlast Trials (2024) is a co-op multiplayer spinoff in the same universe.
